About Fairmount Primary Care Center
Fairmount Primary Care Center provides integrated health care services including dental care to patients in the Fairmount, Spring Garden, and Art Museum neighborhoods of North-Central Philadelphia from its location at 1412 Fairmount Ave, Philadelphia, PA 19130. Fairmount Avenue runs through the center of the Fairmount neighborhood, connecting the Art Museum district to the Northern Liberties corridor and providing access for patients from this densely populated central Philadelphia community.
The center delivers health services in an integrated model that includes dental care alongside primary care, making dental visits accessible and convenient for patients who may receive multiple types of care at the same location. Comprehensive oral examinations assess tooth structure, gum tissue health, and bite alignment, with treatment planning coordinated with the patient's broader health care needs where relevant.
Preventive dental care at the center includes professional cleanings to remove tartar and plaque deposits, fluoride treatments for patients at elevated decay risk, and oral health education to support home hygiene habits between visits. Restorative dental services — including composite fillings for cavity treatment and extractions when necessary — address active oral health problems as they arise. The primary care center approach supports care coordination for patients with systemic conditions, including diabetes and cardiovascular disease, that have meaningful oral health connections.
Fairmount Avenue at 1412 is served by SEPTA bus routes and is walkable from the Spring Garden area, making the center accessible for patients commuting by transit from across Philadelphia. Street parking is available on Fairmount Avenue and the surrounding residential blocks. Fairmount Primary Care Center is a community health resource for Fairmount and North-Central Philadelphia patients who value dental care delivered within an integrated primary care setting.

I am a patient here and I’ve been here for some time . I called for a prescription last Tuesday to my pharmacy . Pharmacy told me it’s expired. I’m like okay . Can yall send a message to the doctor to have it refilled . They said yes. I call the doctors office to get it refilled and was told that the prescribing doctor is no longer there and I’d need to come in for an appointment to get the medicine . I explained 3 times that I’ve been on this medication for years it’s in my chart . The person on the phone said they would send it to Donna because that’s the most recent dr . I had appointment with . Donna called back on Wednesday and I felt pretty good but when I called the pharmacy to pick it up on Saturday they said they don’t have it. Donna informed me she wouldn’t be in the office Monday and Tuesday . I figured well since she sent it already I’d have no issue with another dr just sending the meds . I called today to have the prescription resent and the front desk put a message in . Message was sent at 9:40 am . Now I understand that drs are busy and have lots of parents but I’m out of medicine and it has adverse effects on my health . It doesn’t take more than 5 minutes to click a button to push it to the pharmacy . So I called back again and the receptionist told me that it’ll take 24-48 for the doctor to review my request and that she doesn’t work at this location. I work full time and their five closes at 5pm. So I wouldn’t be able to just run over . I work in jenkintown . I now have to wait until Wednesday which is basically when Donna comes back in . So I couldn’t get the meds because the prescribing dr left, tried to work around that and now I can’t get my meds because Donna is out . These people don’t care about you and there organization is not set up in a way you can get the attention and care you need .DO NOT TAKE YOUR KIDS OR LOVED ONES HERE. I will be switching care offices .
Great
Needed a tooth extraction and I do not have insurance. The rates were trailered to my income, very reasonable. I had a broken tooth and they were able to squeeze me into schedule the next day. Highly recommend.
Customer Service is Horrible! I’ve been a patient for over 20 yrs. I love my PCP however since they’ve changed to an off-site customer service center, my experience with trying to get things resolved has been a nightmare. I called to see if my PCP received a form to fill out and was told by customer service they can’t see if the form was received by my PCP but it takes 2 weeks to have the form filled out. I asked the rep how can a form be filled out if they may have not received it. I explained I needed to verify if the office has it and if she can’t see it in her system then to send a message to my PCP to verify. She then said they can’t no longer send messages to the PCP. What’s the point of this customer service center if they can’t see anything in your file and they can’t contact the doctor’s office on your behalf? They’re there to do absolutely nothing. This type of customer service will make me change offices. This is not the PCP’s fault or any other staff at the office, however I believe the customer service or lack of will cause them to lose patients.
i seriously wish i could give this place zero stars. i just missed an appointment ive been waiting months for because fairmount primary care center refused to answer their phones at the front desk even though it was the middle of the work day, and as a result i was unable to access my medical records i needed for the appointment and was denied my appointment. i can’t even begin to describe how frustrating this was. absolutely horribly unprofessional, please seek care literally anywhere else because these people will not help you in a timely manner.
Friendly and efficient staff. Dr Greta Stewart always takes the time to get back to me and listens to all my concerns. Truly cares for her patients needs and does whatever she can to help. 10/10
I have been seeing Doctor Cox for over a year now and I can say that he is excellent. He takes times to ask me questions about how I’m feeling and whether there are any other things he can help me with today. He was also helpful in setting me up with the portal and the nurse line. Thank you so much.
