I booked Forma Dental for a routine cleaning that had already been verified through my insurance. I left hours later without my teeth cleaned and with nearly $12,000 in proposed dental work. That alone says enough about how this office operates. The incompetence started before I even reached the chair. Marina had already verified my insurance and personally called me to confirm coverage. When I arrived, she still tried to charge me out of pocket until I reminded her of the verification she had already completed. If your own employee verifies coverage, calls the patient about it, then needs the patient to explain that same verification back to her at check-in, your front desk is not functioning properly. Then came Dr. Marianna Pasciuta. While examining my teeth, she looked at their color and asked if I smoked. I don’t. Jumping from tooth shade to “Do you smoke?” was presumptuous, tone-deaf and embarrassingly lazy communication from a dentist. Dr. Pasciuta recommended a deep cleaning. I agreed and made it clear I intended to proceed. She then told me she was going to get the hygienist to clean my teeth. No hygienist came. Instead, Sisi, the office manager, appeared to explain why the cleaning I booked was now being refused. The problem was that she could not explain it. When I told her dentists in other states had performed routine cleanings before scheduling periodontal treatment, her response was, “Standards are different in other states.” Painfully ignorant. I was not asking an office manager for her amateur commentary on dental standards across America. I was asking why Forma verified my insurance, booked me for a cleaning, had its dentist tell me a hygienist was coming, then replaced the hygienist with an office manager who could not coherently explain why the cleaning had suddenly disappeared. After enough circular nonsense, I had Dr. Pasciuta brought back to explain her own decision. She said the routine cleaning “wouldn’t be beneficial” and would mainly address staining. I told her I understood and still wanted the cleaning I booked. I had already agreed to the deep cleaning. They still refused. Forma managed to turn a routine cleaning into an administrative and communication failure. Producing nearly $12,000 in proposed dental work? No problem at all. Routine cleaning covered by insurance: confusion, contradiction, refusal. Five-figure treatment plan: ready immediately. Then there is Forma’s own menu: Botox, fillers, PDO threads, PRP, IV therapy, Kybella, implants and full-mouth reconstruction. This office has the bandwidth to inject faces, administer IVs and operate half a med spa, but ask them to perform the routine dental cleaning you scheduled and suddenly the entire operation falls apart. Their written follow-up was just as defensive. I requested documentation of the visit. They made a point of emphasizing that I requested the routine cleaning, that they refused it, and that their treatment plan had not been “signed or formally accepted,” while also acknowledging that I expressed my intention to proceed with the deep cleaning. That did not read like neutral documentation. It read like self-protective damage control and an attempt to build a paper trail after the fact. If Forma had put the same effort into communicating clearly during the appointment that it put into carefully wording its defense afterward, the visit might not have been such a disaster. I wasted hours at Forma Dental and left with no cleaning, an insurance issue I had to correct myself, a presumptuous smoking question from Dr. Pasciuta, an office manager giving me ignorant and incoherent answers, a hygienist who was supposedly coming but never appeared, and nearly $12,000 in proposed treatment. The office design is polished. The competence, communication and basic dental care were not.
Forma Dental are great. My son was so nervous of dentists but they talk him through everything here. The tools are quiet and they do an excellent job. Highly recommend!!

I’m not from Florida, so finding Forma as a reliable dentist was a huge relief! Maria at the front desk was extremely helpful, and made booking very easy. The entire staff doing my x-rays and helping with my appointment were very efficient and really helped me understand what I needed for the best dental care possible. The text communication post and prior to the appointment made scheduling very easy as well. I’ll definitely be returning to Forma, and I would absolutely recommend the office to anyone looking for a reliable, efficient, professional dental care.

I was genuinely thrilled with my experience there: all staff are extremely courteous and professional, and my exam/cleaning were comprehensive and informative. The facility is beautifully clean and has a modern, welcoming feel. I also enjoyed the use of technology to ensure a proper exam. I’d never seen my teeth in such detail before!
