You do not need a referral from your dentist to be seen at Legacy Oral Surgery, and a fair number of patients at the North Broad Street suite in Elizabeth book themselves in, often after months of avoiding a tooth they already know has to come out. The practice works across the oral and maxillofacial range: routine and difficult extractions, wisdom teeth, bone grafting, sinus lifts, implant placement and full-arch All-on-4, out to oral pathology and traumatic injuries of the teeth and jaws. The practical value of that breadth is that a case which turns out to be more complicated than the referral letter suggested does not have to move again to a third office.

A sinus lift is worth explaining, because it is the reason so many upper-back implant plans take an extra season to complete. Above your upper molars sits the maxillary sinus, an air-filled space. When an upper molar has been missing for a while, the bone beneath that sinus thins from disuse, and there is often not enough vertical height left to anchor an implant securely. The procedure adds height. Working through a small window in the side of the jaw, the surgeon carefully lifts the thin membrane lining the sinus floor upward and packs grafting material into the space created underneath it. Over roughly six to nine months your own bone gradually replaces that graft. The implant is then placed into it, sometimes at the same appointment as the lift if enough native bone remains to hold the implant steady, and sometimes only once healing is complete. Which of the two applies is decided from the scan, not from preference.

Sedation is offered across the full range, from local anesthetic alone through nitrous oxide, intravenous sedation and general anesthesia, all administered on site, so the choice is a real one rather than a single option presented as though it were the only one. Planning runs off cone beam CT imaging taken in the office, measuring bone height and nerve position in three dimensions before anything is committed to.

The suite is on the second floor at 700 North Broad Street, and the working week has a gap in the middle of it: Monday, Tuesday, Thursday and Friday, nine to six, with Wednesday and the weekend closed. That shape is better planned around than discovered afterwards. A Monday or Tuesday extraction puts an open day immediately after the swelling peaks, when a stitch that has come loose or a socket that feels wrong can actually be looked at. A Friday one does not, and the next working morning is the following Monday. If you are having surgery late in the week, ask before you leave what the office wants you to do about a problem on Saturday, and go home with a written number rather than a general one.

Do I need a referral from my dentist to book here?
No. Many patients arrive through a general dentist, but the practice accepts appointments directly, so you can schedule a consultation yourself. Bringing any recent X-rays your dentist has taken is useful and can save a repeat image, though the office has cone beam CT on site if three-dimensional imaging is needed for planning an extraction, a graft or an implant.
What sedation options are available for surgery?
Four levels, all given in the office: local anesthetic on its own, nitrous oxide for mild anxiety, intravenous sedation which leaves most patients with little memory of the procedure, and general anesthesia. The right choice depends on the complexity of the surgery, your medical history and how anxious you are. Anything beyond local anesthetic requires fasting beforehand and someone to drive you home afterward.
How long after a sinus lift can the implant be placed?
It depends on how much of your own bone remains beneath the sinus. If there is enough to hold an implant steady at placement, the lift and the implant can sometimes be done in one appointment. If not, the graft is left to mature for roughly six to nine months while your bone replaces it, and the implant follows afterward. The scan settles which route applies.

I took my 7-year-old son to Legacy Oral Surgery for a tooth extraction. The office is nice, clean, very organized, and the staff is very friendly. Overall, we had a good experience. The only thing that upset us was that I asked the doctor if we could have my son’s tooth back because kids love to leave their teeth for the Tooth Fairy. Unfortunately, the tooth was accidentally thrown away. When my son woke up from the anesthesia, I told him they couldn’t find it because it had been discarded. He cried the entire ride home, and it broke my heart. I know it’s just a tooth to some people, but to a child, it means a lot. I think the office just needs to pay a little more attention to details like this when working with children. It may seem like a small thing, but it can make a big difference in a child’s experience. I would still recommend the office, but I hope they use this feedback to make future visits even better for their youngest patients.

I want to provide my review... They did assist me on my oral surgery (wisdom teeth + additional one teeth removal). I was in good hands and best recovery. I am grateful for that BUT THEIR CUSTOMER SERVICE SUCKS. I have been waiting for my refund on my implant which I declined due to misleading information on the procedure and recovery. IT HAS BEEN OVER 2 MONTHS AND NO REFUND ON THEIR PART. I keep calling their office for an update, it's always "let me know to someone in the department" or "it has been sent out & wait for 7 to 10 days". This is not acceptable customer service. plus I tried to refer to my friend for her wisdom teeth removal and they didn't want to explain to her why her insurance won't cover the procedure. Luckily, she contacted her insurance and it's out of network. But how could no one in that office mention anything to you about it?

I had a great experience with this practice. I was having problems making an appointment with other oral surgeon practices because when I called them they would not verify if they would take my insurance and they would not run my insurance information until I came in for a consultation. I then came across Legacy oral surgery on google and when I called the Staten Island office I was very lucky to get Cathy on the phone. She was great. She took the time out to run my insurance and she insured me that the procedure would be covered. She was then able to make an appointment for the next day and the procedure went great. The procedure was performed by Dr. K and he was also amazing. I will certainly recommend this practice to friends and family and will definitely tel them to ask for Cathy when they call. I can’t thank her enough for how she helped me out.
