About Park Duvalle Community Health Center Inc
Park Duvalle Community Health Center Inc. is located at 3015 Wilson Avenue in Louisville, Kentucky — a community health center in the Park DuValle neighborhood of west Louisville, Jefferson County ZIP 40211. Park DuValle is a historically African American neighborhood in the west Louisville corridor, bounded by the Western Parkway and Algonquin Parkway and known for the Park DuValle public housing redevelopment of the late 1990s that replaced the older Cotter and Lang public housing with mixed-income housing development. Wilson Avenue in ZIP 40211 runs through the west Louisville neighborhood fabric, and 3015 Wilson Avenue places the Park Duvalle Community Health Center in the heart of the neighborhood it serves. Community health centers in historically underserved urban communities like Park DuValle operate as FQHCs (Federally Qualified Health Centers) or with similar community health center models, providing comprehensive primary care — medical, dental, behavioral health — for uninsured, Medicaid, and low-income community residents at sliding-fee-scale payment rates adjusted to income. West Louisville's ZIP 40211 demographic reflects the historic disinvestment patterns of Louisville's west-side neighborhoods — lower household incomes, higher Medicaid rates, and a patient population that relies heavily on community health center infrastructure for healthcare access.
Park Duvalle Community Health Center provides comprehensive health services — including dental care — for residents of west Louisville and the surrounding Jefferson County communities. Dental services at community health centers in this model typically include preventive dental care (examinations, cleanings, fluoride treatments, radiographs), basic restorative procedures (tooth-colored fillings), dental extractions for symptomatic or unrestorable teeth, and emergency dental treatment for patients with acute dental pain. The sliding-fee-scale payment model enables patients to receive dental care at costs adjusted to their income and family size, based on federal poverty level guidelines — from zero cost for the poorest patients to full fee for patients at or above 200% FPL with income. West Louisville's community health center network includes Park Duvalle Community Health Center as a key dental and medical access point for neighborhoods that have historically faced barriers to private-sector healthcare access.
Wilson Avenue's west Louisville position provides access from the Western Parkway and Algonquin Parkway corridors running through Louisville's west side, from the TARC bus system serving west Louisville's transit-dependent patient population, and from the I-264 (Watterson Expressway) interchange connecting west Louisville to the broader Jefferson County highway network. Louisville's full dental specialist network is accessible from ZIP 40211 via the downtown and Jefferson County highway grid for patients requiring orthodontic, oral surgery, or periodontal specialist care.
For west Louisville and Jefferson County patients seeking sliding-fee-scale community dental and health care in the Park DuValle neighborhood, Park Duvalle Community Health Center Inc. at 3015 Wilson Avenue provides FQHC-model health access in one of Louisville's historically underserved west-side communities.

Positive energy spreads positive energy, Negative energy spreads Negative energy This place will put you in a bad mood. They act like i cane to they houae without calling first..If you go here DO NOT GO WHEN THEY FIRST OPEN, EMPLOYEES LATE, MAD YOU SHOWED UP , THEY WILL IGNORE YOU, AND TERRIBLE ATTITUDES LIKE THEY HATE THEIR JOBS or YOU BOTHERING THEM. SLOW, RUDE, AND UNCARING.. if you check them on the unprofessionalism, you the problem. Im ready for A.I to takeover the medical field. Throw the whole building away every department
I had a very disappointing experience during a telehealth appointment with Nikeaa Bullock. The appointment appeared to be going normally until there was confusion regarding my location after I informed her that I had moved to Cleveland, Ohio for my job. At that point, the conversation became tense very quickly. I felt interrupted multiple times and was told to stop talking while I was attempting to explain the situation. I was also told that I was being aggressive, which was upsetting because I was making a conscious effort to remain calm and respectful throughout the conversation. I understand that telehealth providers must follow licensing requirements, and if there were restrictions on continuing the appointment, I would have appreciated a clearer and more professional explanation. Instead, I felt dismissed and treated as though I was a problem rather than a patient trying to understand what was happening. The interaction left me feeling distressed and unsupported, and I ultimately ended the call in tears. Based on my experience, I will not be returning for care.
The office is terribly rude , and they never answer the phone . And when they do it’s not a polite greeting . Everyone there acts like they don’t want to do their job and that’s exactly that . No patient should have trouble getting their records or having trouble gettting in reach with the office . I am a medical assistant myself and with THAT attitude would never be acceptable. They always give the run around and they’re just not considerate of people who may actually need to speak with them urgently about a situation .
Doctors offices are suppose to be a lovely place for everyone to get well and Park Duvalle is a lovely relevance place!Thank you Park Duvalle for having transportation and coming to get me when I couldn’t drive anymore: for every visit and dropping me off back home! And thanks especially for being a doctors office I can get my medicine prescribed, pediatrics for my 16 year old, dentist office and my mental doctor too! All in 1 building!!! I love Louisville 😘Thank you nurses for being nice and getting us ready for our Doctors (our dependable life strongholds) and Seriously Thanks to you nurses for being courteous, courageous and all around lovely for our horrible pains we have!😘 I Love You Park Duvalle Doctors for going to school, graduating, and passing the boards to become our doctors! 😘 I’ve been with Park Duvalle as a patient, for over 5 years and I am HAPPY 😃!
Glad we have a clinic in our neighborhood, and the staff is fine. But NONE of the numbers work, nobody ever answers, and when I get a text telling me to call to refill my prescription the number is always disconnected.
I recently started to bring my kids back here. Im pleasantly surprised that it looks nicer and the people seem to be more friendlyer. The only negative thing I have is, the constant need to ask about my housing(if Im on subsidized) I have never experienced that before, until now. So, I'm guessing they maybe getting some sort of incentive; if they have patients that receive housing. Other than that it's a fantastic pediatric Doctors office. I would recommend.
