Suite 4 at 456 North 5th Street sits in Philadelphia's Northern Liberties just off the Spring Garden Street corridor, minutes from the Vine Street Expressway ramps and a short walk from Spring Garden station. That is a practical location if you are being driven home after sedation, which is how most patients leave an oral surgery appointment; parking here is easier than in Center City but still largely on-street. This is a single-surgeon practice led by Dr. Cyndi Nguyen, who took her dental degree at Louisiana State University and practised general dentistry for close to thirteen years before training in oral and maxillofacial surgery.

Extractions are the bread and butter, and the difficult ones are the point. A wisdom tooth that has not erupted sits under bone and gum, often angled against the tooth in front and sometimes with roots curving around the inferior alveolar nerve. Removing it involves lifting a flap, taking away a small amount of bone and frequently sectioning the tooth so the pieces come out along their own paths rather than forcing the whole root mass through a narrow opening. The same logic applies to entangled or fused roots on older molars. Cases a general dentist passes on are usually referred for exactly these reasons.

Implant work runs from a single tooth to a full mouth, supported by a 3D cone beam scanner for planning, an intraoral digital scanner, in-house milling and smile-design software, so a proposed restoration can be designed and produced without waiting on an outside laboratory. Bone and soft tissue grafting, sinus lifts and corrective jaw surgery are handled here too, and complex cases other offices have declined are a stated focus.

Bone grafting adds time rather than complexity from your side. Graft a socket on the day of extraction and there is typically a three-to-six-month wait while it knits into solid bone; the implant follows after that, on a schedule your own healing sets rather than the one you would prefer. Hours are 8:00 a.m. to 5:00 p.m. Monday through Thursday and to 2:00 p.m. on Friday. Dr. Nguyen also teaches and does volunteer surgical work overseas and with Native American communities.

What kinds of cases does this practice take on that a general dentist would not?

How long after a graft before an implant can be placed?
Usually three to six months, depending on the size of the defect and the type of graft. Bone placed at the time of extraction to preserve a socket matures faster than a large ridge augmentation or a sinus lift. The graft has to be replaced by your own living bone before it can hold an implant under load, and that is biology rather than scheduling. Rushing the interval is a common reason an implant fails to integrate.
Will I be asleep for the procedure?
Sedation options are available and are chosen according to the procedure, your medical history and how anxious you are, from local anaesthetic alone through to deeper sedation for longer surgery. Under sedation you are not under general anaesthesia in the hospital sense, but you will be drowsy and you will not be driving. Arrange a lift home in advance and follow the fasting instructions the office gives you, or the appointment will be rescheduled.

Absolutely fantastic experience. Everyone was so kind and informative. Im petrified of the dentist, but i had to get all 4 wisdom teeth removed, so i was referred here. Dr. Cyndi Nguyen is the best oral surgeon EVER!!! My 20 yr old son was referred here by his dentist. First, after looking at the xrays, she decided to only take 2 wisdom teeth. I really appreciated that she didn't take out teeth that didn't need to come out. That's real integrity. Second, she patiently answered all our questions and allowed me to come back the day of surgery to be with my son until he was ready to go under. Third, he was out in less than a half hour and she did an amazing job!! He didn't anything other than extra strength Ibuprofen and Tylenot. And it healed so quickly- we couldn't believe it. She is the best oral surgeon and I'm defintely taking my other son here for his wisdom teeth. Thank you so much Dr. Cyndi Nguyen- I see why you've received all of these impressive awards!

I had consult with other oral surgeon and told me I had to wait 8 weeks for an appointment for a tooth with excruciating paid. A coworker recommended Dr. Nguyen. I called them up! They got me in within 2 days for a same day surgery. I was reluctant to get wisdom teeth pulled that weren’t bothering me, but Dr. Nguyen didn’t just recommend they come out , she out a little camera back there and showed me exactly why they were going to cause issues in the future. I was placed under anesthesia, 4 extractions, and woke up pain free! 10/10 I would absolutely return to her only. Not to mention her beautiful office . Wow ! Hands down great beautiful Doctor. Her assistant and financing team are very friendly, work diligently, and quick and with no delay. I love it here !!

Dr Cyndi Nguyen and her entire Staff provide Excellent Patient Care! ! The office is a very comfortable and pleasant environment. I received great customer service beginning with my initial call for an appointment, which I received that same week !
